Faith Formation 2023-2024 School Year

Please complete this registration form and mail or drop off the registration fees owed to the parish that your child/children will receive religious instruction to secure your place in class. Please note, if you register your children at multiple locations, please send your payment to the faith formation office where child #1 is registered.

Religious Education Fees
1 child: $50 per family
2 children: $75 per family
3 children: $95 per family
4 or more children: $100 per family

Additional Sacramental Fees
1st Reconciliation (2nd Grade): $25 per child
1st Communion (3rd Grade): $25 per child
Confirmation (10th Grade): $70

God’s Family (for individuals w/disabilities & the family): $50 per family

*If you are registering multiple children for BOTH regular faith formation classes & the God’s Family program, please note that you need to pay the fees for both programs (the God’s Family fee is in addition to regular faith formation fees)
*If you are registering 1 or more special needs individuals for the God’s Family program only, you need only to pay the $50 God’s Family program fee

Registration forms and fees are due by August 1, 2023

Registration Late Fee: ADD $15.00 if registering after August 1, 2023
